This SUBSCRIPTION covers mechanical, electrical, and electronic components on the GOLF CART shown above subject to the exclusions listed in this SUBSCRIPTION.
In the event of a malfunction, defect, or failure of a COVERED PART, not listed on the exclusions, due to faulty workmanship or materials supplied by the original Golf Cart manufacturer, WE will repair or replace the COVERED PART, subject to terms, conditions and limitations specified herein less the DEDUCTIBLE amount shown, which YOU are responsible to pay.
This SUBSCRIPTION does not cover: OEM Warranty covered Repairs and Parts; BREAKDOWNS resulting from abuse, Pre-existing conditions, Commercial Use of GOLF CART; BREAKDOWNS due to failure to perform or negligently performed maintenance service and/or repairs as recommended by the manufacturer. Maintenance parts and services are NOT covered under this SUBSCRIPTION.
I. Definitions & Key Terms
ADMINISTRATOR, WE, US or OUR: means the ADMINISTRATOR, provider, and obligor of this SUBSCRIPTION - DealerAdmin.io, LLC, 8940 Fourwinds Drive Suite 610, Windcrest, TX 78239 who is the Obligor to this SUBSCRIPTION.
BREAKDOWN: The failure of any COVERED PART to perform the function for which it was intended.
SUBSCRIPTION: means this GOLF CART Service SUBSCRIPTION.
SUBSCRIPTION HOLDER, YOU, or YOUR: means the purchaser or holder of this SUBSCRIPTION.
COVERED PART(S): Any part that is factory installed original equipment on YOUR GOLF CART or like replacement parts meeting the manufacturer's specifications and which is not excluded from this SUBSCRIPTION coverage.
DEDUCTIBLE: In the event of a BREAKDOWN of a part, repaired or replaced under the terms and conditions of this SUBSCRIPTION, the purchaser will be subject to pay the applicable DEDUCTIBLE as specified above.
GOLF CART, YOUR GOLF CART: means the GOLF CART which is described above.
Insurance Company Statement
The performance and obligations of the provider for this SUBSCRIPTION are guaranteed and insured by a Reimbursement Insurance Policy issued by Verisure Insurance Company (INSURER), Tel: (800) 436-9498. In the event WE cease to operate, become bankrupt or fail to pay YOUR claim within sixty (60) days after proof of loss has been filed, YOU may make a direct claim against Verisure Insurance Company.
Limit of Liability
OUR liability under this SUBSCRIPTION is limited to: (1) the lesser of the reasonable cost to repair or the cost to replace a part within the covered component with a like kind and quality part which may include, new, serviceable used, or remanufactured parts as customarily used in the Golf Cart industry, and reasonable installation labor charges based on the national average labor rate, as needed to correct the actual cause of a covered component BREAKDOWN.
The SUBSCRIPTION aggregate total of all pending and paid claims cannot exceed the original purchase price of the covered GOLF CART.
II. Coverage / Protection Plans
This SUBSCRIPTION is a Subscription between You and the Provider Obligor; it is not an insurance policy. All obligations and liabilities for repairs covered by the Subscription are those of Provider Obligor.
EXCLUSIONARY COVERAGE - BREAKDOWN COVERAGE: If You have bought the Exclusionary Coverage as indicated on the Schedule Page, We will pay or reimburse You for Reasonable Costs to Repair or replace any Breakdown of all mechanical or electrical parts or components on Your Golf Cart, except those listed under EXCLUSIONS, less any Deductible.
Additional Benefits Under This Coverage
GOLF CART PICKUP: In the event of a Breakdown of a covered part, We will reimburse You up to one-hundred dollars ($100.00), for reasonable pickup and delivery charges associated with having Your Golf Cart taken to a Repair Facility. We will require an itemized repair work order and all repairs must be pre-authorized prior to repair(s) being performed.
ON-SITE SERVICE BENEFIT: In the event of a Breakdown of a covered part, We will reimburse You up to fifty dollars ($50.00), for on-site service charges for covered/authorized Repairs. We will require an itemized repair work order and all repairs must be pre-authorized prior to repair(s) being performed.
Roadside Assistance Option
24/7 Emergency Roadside Assistance: 1-888-655-1196
Provides emergency road services for non-accident related incidents, where failure is due to a defect in material or workmanship.
Benefit Limits: For the TERM OF YOUR SUBSCRIPTION, the listed sign and drive benefits are available 24 hours a day / 365 days a year. The following non-accident related services are available for covered BREAKDOWNS up to a maximum benefit of $100.00 per incident:
- Towing
- Jump Starts
- Flat Tire Changes (using YOUR inflated spare)
- GOLF CART Fluid Delivery (cost of GOLF CART fluids extra)
- Lockout Assistance (key cutting/replacement extra)
- Concierge Services (courtesy help and emergency phone call support)
** Benefits are limited to "cash call with reimbursement" service only for the first forty five (45) days of coverage. **
III. Exclusions & What's Not Covered
MAINTENANCE AND PARTS NOT COVERED: Normal maintenance items/repairs, such as cleaning parts, engine tune-ups and front-end alignments, are not covered. Adjustments and alignments to covered parts are not covered. In addition, this SUBSCRIPTION provides no benefits or coverage for:
- Repair/replacement without prior authorization from the Administrator
- Repair/replacement when no Breakdown has occurred
- Breakdown caused by contamination, foreign objects, improper fluids
- Breakdown from continued operation or failure to take reasonable precautions
- Adjustments, repositioning, refitting, realigning, cleaning
- Modifications not performed or recommended by manufacturer
- Lift Kits, Oversized Tires, accessories not installed by Manufacturer
- Commercial Use of GOLF CART
- Competitive driving, racing, neglect, abuse, misuse
- Emergency Golf Carts, taxi, limousine, livery, utility Golf Carts
- Golf Carts without valid VIN/Serial numbers
- Collision, theft, fire, flood, weather damage, acts of nature
- Damage from improper or over-charging of Golf Cart batteries
- Improper tow or storage
- Pre-existing conditions
- Breakdowns not occurring in the United States or Canada
IV. Your Responsibilities & Obligations
In order to keep Your SUBSCRIPTION valid, You must follow the maintenance procedures below:
- Your Golf Cart must be serviced receiving all scheduled maintenance as recommended by the Manufacturer in the Owner's Manual
- Keep receipts which verify the Golf Cart Identification Number/Serial Number, work orders and other documents showing date, description, mileage and services performed
- Hand Written receipts will not be accepted
- Ensure oil warning light/gauge and temperature warning light/gauge are functioning before driving
- Shut down engine immediately when warning lights indicate a problem
If You experience a Breakdown, You agree to:
- Use all reasonable means to protect Your Golf Cart from further damage
- Notify the Administrator as soon as possible
- Authorize the Repair Facility to perform necessary diagnostic work
- Furnish proof of regular maintenance if requested
- Obtain authorization from the Administrator prior to beginning any repairs
V. What To Do If You Need Repairs
To ensure coverage under the terms of this SUBSCRIPTION, authorization must be obtained prior to repair.
Call the toll free claims number: 1-800-436-9498 between 8:00 a.m. - 5:00 p.m. CT Monday ā Friday.
Emergency Repair Instructions: In the event that a Breakdown occurs when the Administrator's office is closed and emergency repairs are necessary, You may commence emergency repairs without securing prior authorization. However, You or the authorized service representative must notify the Administrator as soon as the office reopens and submit documentation no later than the next business day after the Breakdown.
VI. General Provisions
1. Subscription Term: The Subscription Term is Monthly. The Subscription begins on the Subscription Date as shown on the Schedule Page. The Subscription ends 30 days following the last payment received by OUR ADMINISTRATOR.
2. When And Where You Are Covered: You are covered when this Subscription is issued to You. This Subscription applies only to repairs occurring within the continental United States of America, Alaska, Hawaii, and Canada.
3. Limit of Liability/Aggregates: Our liability for the term of this SUBSCRIPTION is limited to the J.D. Power NADA Value of the Golf Cart at the time of the repair.
VII. Dispute Resolution ā Arbitration
This Subscription requires binding arbitration if there is an unresolved dispute between You and Us concerning this Subscription. Under this Arbitration provision, You give up Your right to resolve any dispute by a judge and/or jury. You also agree not to participate as a class representative or class member in any class action litigation, any class arbitration or any consolidation of individual arbitrations. The arbitration shall be governed by the Federal Arbitration Act (9 U.S.C.A. § 1 et. seq.) and rules of the American Arbitration Association (www.adr.org). The laws of the state of Texas govern all matters arising out of or relating to this Subscription.
VIII. How To Transfer This Subscription
This SUBSCRIPTION is NOT TRANSFERABLE.
IX. How To Cancel This Subscription ā Including Refunds
YOU may cancel this SUBSCRIPTION at any time.
All cancellation requests made within twenty (20) days of the purchase date of the SUBSCRIPTION will be eligible for a full refund.
All cancellation requests made after twenty (20) days of the purchase date: Will be pro-rated by the time from the purchase/renewal date of the SUBSCRIPTION, less claims paid.
WE may cancel at any time if:
- YOUR GOLF CART is totaled
- YOUR GOLF CART is used in a manner not covered by this SUBSCRIPTION
- The charge for the SUBSCRIPTION is not paid or renewed monthly
- YOU employed intentional misrepresentation in obtaining the SUBSCRIPTION
- YOU employed intentional misrepresentation in the submission of a claim
- YOUR GOLF CART does not have a valid manufacturer VIN or Serial Number
- YOUR GOLF CART's title is branded as salvage, junk, rebuilt, totaled, or flood damaged
- YOUR claim aggregate has reached the original GOLF CART Purchase Price
Entire Subscription
This Subscription represents the entire SUBSCRIPTION between You and Us. No person has the authority to change this Subscription or to waive any of its provisions. Customer Notice
- This Agreement is between You and Provider/Obligor and describes the Coverage Level for the Term Selected above.
- You are applying for a Golf Cart Service Contract ("Agreement") providing the coverage listed above for the vehicle listed above.
- You acknowledge Your understanding of and agree to the Terms and Conditions of the Agreement including Section 7 General Provisions - Dispute Resolution ā Arbitration.
- The purchase of this Service Contract is not required to obtain financing or to purchase or lease this Golf Cart.
- You are required to obtain authorization prior to beginning any repairs covered by this Service Contract by calling 800-436-9498.
Service Contract
This Agreement is a Service Contract between You and the Provider/Obligor; it is not an insurance policy. All obligations and liabilities for repairs covered by the Service Contract are those of Provider/Obligor. This Agreement describes the coverage You will have under Your Service Contract. In return for payment by You of the Service Contract Price and subject to all the terms of this Service Contract, We agree with You as follows:
Key Terms
Obligor, Provider, We, Us, Our: DealerAdmin.io, LLC, 8940 Fourwinds Drive, Suite 610, Windcrest, TX 78239, 800-436-9498.
You or Your: The purchaser identified under the Customer Information section.
Vehicle or Covered Vehicle: The Golf Cart, LSV described in the Vehicle Information section.
Agreement Term: The number of Months or Miles the Agreement is in force provided the appropriate fee is paid and received by Us.
Breakdown: The failure of any original or replacement part covered by the Agreement when: (1) The covered part fails to perform as originally designed under normal usage, (2) Failure is due to a material defect in workmanship, (3) Failure is outside manufacturer's tolerance or specifications, (4) Normal wear and tear exceeds manufacturer's specifications, and (5) The Vehicle has received all required manufacturer-recommended maintenance.
Limit of Liability/Aggregate: The total amount this Agreement will pay toward a repair or over the life of the Agreement.
Deductible: The amount You must pay for covered repairs per visit. The standard Deductible is $50.00. If repairs are done by selling dealer or affiliated dealership, the deductible is $0.
Pre-Existing Conditions: Any Vehicle determined to have a failure due to a condition which existed prior to the Agreement purchase date.
Repair Facility: The Dealer/Seller or a licensed Repair Facility located in the continental United States, Alaska, Hawaii or Canada, approved by the Administrator.
Coverages - Exclusionary Coverage
BREAKDOWN COVERAGE: We will pay or reimburse You for Reasonable Costs to Repair or replace any Breakdown of all mechanical or electrical parts or components on Your Vehicle, except those listed under EXCLUSIONS & WHAT'S NOT COVERED, less any Deductible, in accordance with the GENERAL PROVISIONS section.
Reimbursement amounts for replacement parts or components may be based on new, non-OEM, remanufactured or used parts at the Administrator's sole discretion. Parts will be reimbursed up to manufacturer's suggested list price.
Optional Coverages / Surcharges
EMERGENCY SERVICES BENEFIT: In the event of a failure of a mechanical or electrical component, when towing is necessary, we will reimburse you when the covered Unit is towed to the nearest authorized service facility up to $100 per occurrence, once per 72 hours, maximum of 5 occurrences over the life of the contract.
ROADSIDE ASSISTANCE:
- Battery Boost - If battery failure occurs, a jump-start will be applied
- Gasoline, Oil, Fluid & Water Delivery Service - Emergency supply delivered (you pay for fluids)
- Towing and Road Service - Up to $100 for towing and/or emergency road service labor
RENTAL/ALTERNATIVE TRANSPORTATION: In the event of a covered mechanical or electrical failure, WE will reimburse YOU for a RENTAL CART and/or ALTERNATE TRANSPORTATION (Uber/Lyft/Commercial Rideshare) while your Golf Cart is being repaired. Maximum daily benefit: lesser of $50.00 or actual expense. Total days per occurrence limited to repair time up to 10 days maximum or lifetime benefit of $500.00.
Additional Benefits Under This Coverage
VEHICLE PICKUP: In the event of a Breakdown, We will reimburse You up to $100.00 for reasonable pickup and delivery charges. Itemized repair work order required; all repairs must be pre-authorized.
ON-SITE SERVICE BENEFIT: In the event of a Breakdown, We will reimburse You up to $50.00 for on-site service charges for covered/authorized Repairs. Itemized repair work order required; all repairs must be pre-authorized.
Exclusions & What's Not Covered

- B. Software/Firmware Updates and reprogramming; GPS software updates
- C. Repair/replacement when no Breakdown has occurred
- D. Breakdown caused by contamination, foreign objects, improper fluids, or lack of manufacturer's required maintenance
- E. Breakdown from continued operation or failure to take reasonable precautions
- F. Repair of valves or rings to raise engine compression
- G. Adjustments, repositioning, refitting, realigning, cleaning, squeaks, rattles, water leaks, wind noise
- H. Parts not necessary for covered Breakdown repair
- I. Breakdown caused by non-manufacturer modifications
- J. Lift Kits, Oversized Tires, accessories not installed by Manufacturer
- K. Commercial Use (unless Optional Commercial Surcharge selected) including emergency vehicles, taxi, limousine, livery, RV Parks, Apartments, Golf Course vehicles, construction sites
- L. Competitive driving, racing, neglect, abuse, misuse
- M. Collision, glass breakage, fire, theft, explosion, earthquake, windstorm, hail, flood, rust, corrosion, contamination, malicious mischief, riot, lightning, environmental fallout, freezing
- N. Damage from improper or over-charging of Vehicle batteries
- O. Damage from improper tow or improper storage
- P. Breakdown if manufacturer has voided warranty on Your Vehicle
- Q. Loss of use, time, profits, savings, inconvenience, commercial loss, or consequential damages
- R. Liability to third parties other than Administrator-approved repairs
- S. Liability for property damage, injury or death arising from Vehicle operation
- T. Costs covered by repairer/supplier guarantee, faulty repair work, manufacturer warranty, or manufacturer recalls
- U. Parts not covered by original manufacturer warranty
- V. Parts and accessories not supplied by Original Equipment Manufacturer
- W. Breakdown not occurring in the United States or Canada
- X. Pre-existing conditions - any Breakdown that existed prior to Service Contract purchase date

- Your Vehicle must be serviced receiving all scheduled maintenance as recommended by the Manufacturer in the Owner's Manual
- Keep receipts which verify VIN/Serial Number, work orders showing date, description, mileage and services performed
- Hand Written receipts will not be accepted
- Ensure oil warning light/gauge and temperature warning light/gauge are functioning before driving
- Shut down Golf Cart immediately when any warning lights/gauges indicate a problem
If You experience a Breakdown, You agree to:
- Use all reasonable means to protect Your Vehicle from further damage
- Notify the Administrator as soon as possible
- Authorize the Repair Facility to perform necessary diagnostic work (WE WILL NOT PAY FOR DIAGNOSIS CHARGES FOR REPAIRS NOT COVERED)
- Furnish proof of regular maintenance if requested
- Reserve for the Administrator the right to refer Your Vehicle to the Dealer/Seller
- Allow the Dealer/Seller or Administrator to examine Your Vehicle if requested
- Obtain authorization from the Administrator prior to beginning any repairs
How to File a Claim
If Your Vehicle is within fifty (50) miles of the Dealer/Seller, You must deliver Your Vehicle to the Dealer/Seller. If more than fifty (50) miles away, call (800) 436-9498 for instructions before delivery.
To ensure coverage, authorization must be obtained prior to repair. Call toll free: (800) 436-9498 between 8:00 a.m. - 5:00 p.m. C.T. Monday ā Friday.

Parts must be available for inspection when the Administrator's office reopens.
General Terms and Conditions
1. Service Contract Term: The Term is the Months and/or Miles shown on this Agreement. The Contract begins on the Purchase Date and ends when the expiry date or mileage is reached.
2. When And Where You Are Covered: You are covered when this Service Contract is issued to You. This Contract applies only to repairs occurring within the continental United States of America, Alaska, Hawaii, and Canada.
3. Your Help And Assistance: Your help is required if We ask You to help Us enforce Your rights against any manufacturer or Repair Facility responsible for covered repair Costs.
4. If You Have Other Coverage: If You have any other coverage, We will pay only the amount in excess of that coverage, subject to the limits of this Service Contract.
5. Limit of Liability/Aggregates: Our liability for the term of this agreement is limited to the J.D. Power NADA Value of the vehicle at the time of the repair.
6. Subrogation: If We pay for a loss, We may require You to assign to Us Your rights of recovery against others.
7. Dispute Resolution ā Arbitration
This Service Contract requires binding arbitration if there is an unresolved dispute between You and Us concerning this Service Contract. Under this Arbitration provision, You give up Your right to resolve any dispute by a judge and/or a jury. You also agree not to participate as a class representative or class member in any class action litigation, any class arbitration or any consolidation of individual arbitrations. A group of three arbitrators will give a decision after hearing Your and Our positions. The decision of a majority of the arbitrators shall be final and binding and cannot be reviewed or changed by, or appealed to, a court of law.
The arbitration shall be governed by the Federal Arbitration Act (9 U.S.C.A. § 1 et. seq.) The rules of the American Arbitration Association (www.adr.org) will apply. The laws of the state of Texas govern all matters arising out of or relating to this Service Contract.
8. How To Transfer This Service Contract
This Service Contract may be transferred to subsequent purchaser directly by You within thirty (30) days from the date of sale upon payment to the Administrator of a fifty-dollar ($50) transfer fee. This Contract may not be transferred to any entity in the business of selling or leasing motor vehicles. In the event of Your death, the benefits will be available to Your spouse or legal representative.
9. How To Cancel This Service Contract ā Including Refunds
Cancellation By You: You may cancel at any time. Contact the Selling Dealer or provide written notice to the Administrator with a copy of Your Service Contract.
Cancellation By Lienholder: If financed, the Lienholder may cancel if You default or if Your vehicle is declared a total loss or repossessed.
Cancellation By Us: We will mail written notice at least thirty (30) days prior to cancellation. We may cancel within ninety (90) days for any reason. After ninety (90) days, We may cancel for material misrepresentation/fraud, non-payment, or use not covered by this Contract.
How Refunds Are Calculated: If canceled within thirty (30) days and no claim has been incurred, a 100% refund will be made. After thirty (30) days or if a claim has been incurred, a pro-rata refund will be calculated based on unused Months or Miles (whichever is greater), less an administrative fee of $50 and any claims paid.
10. Entire Service Contract
This Service Contract represents the entire agreement between You and Us. No person has the authority to change this Service Contract or waive any of its provisions. No other written or oral statements apply. We shall not be responsible for any Loss arising out of the unauthorized access or use of any system, software, hardware, or firmware.
State Disclosures
The following Special State Requirements apply if this Agreement was purchased in one of the following states:
ALABAMA: Return within 20 days of mailing (or 10 days of delivery at sale) for full refund if no claim made.
ARIZONA: Selling Dealer will perform services in a timely, competent, workmanlike manner.
ILLINOIS/IOWA: Cancellation fee will be lesser of 10% of price or $50.00.
MISSOURI: Written confirmation within 15 days; 10% penalty per month if refund not paid within 30 days.
NEVADA: Pre-existing conditions not covered. Provider may not cancel. Non-renewable.
NEW MEXICO: Provider cannot cancel. 10% penalty per month if refund not paid within 30 days of return.
TEXAS: Complies with Texas Code 1304.004(b)(6). Contact Texas Department of Licensing and Regulation, P.O. Box 12157, Austin, TX 78711 (512-563-6599).
UTAH: Subject to limited regulation by Utah Insurance Department. Coverage not guaranteed by Utah Property and Casualty Guaranty Association.
WISCONSIN: Subject to limited regulation by Office of Commissioner of Insurance.
